Use of the Site does not create a doctor-patient relationship between you and Living Tree Medical Group. Appointment availability, service offerings, and clinic hours vary by location and are subject to change. Scheduled appointments may be canceled or rescheduled by contacting the applicable clinic location. Repeated no-shows or late cancellations may result in fees or discontinuation of scheduling privileges, in accordance with our internal policies.
You agree to use the Site only for lawful purposes and in a way that does not infringe the rights of, restrict, or inhibit anyone else’s use and enjoyment of the Site. You agree not to:
Living Tree Medical Group offers an SMS communication program for patients who provide consent. The following terms apply to all patients who opt in to receive text messages from us.
By providing your mobile phone number to Living Tree Medical Group during patient intake, through our website forms, or by other written or electronic means, and by affirmatively opting in, you consent to receive SMS text messages from Living Tree Medical Group. Messages may include appointment reminders, care instructions, feedback and review requests, health-related follow-ups, and other communications related to your care.
Consent to receive SMS messages is not a condition of receiving medical services from Living Tree Medical Group.
Message frequency varies based on your interactions with us, your appointment history, and the programs you have opted into. Typical patients receive between 1 and 8 messages per month.
Message and data rates may apply. Your mobile carrier’s standard rates for text messages will apply to all SMS communications you send to or receive from Living Tree Medical Group. Contact your carrier for details on your specific plan.
You may cancel your SMS subscription at any time by replying STOP, UNSUBSCRIBE, CANCEL, END, or QUIT to any text message you receive from us. After you send any of these messages, you will receive one final confirmation message confirming your unsubscribe request. After that, you will no longer receive SMS messages from Living Tree Medical Group unless you opt back in.

Our SMS program is compatible with all major U.S. wireless carriers, including AT&T, Verizon, T-Mobile, US Cellular, and others. Carriers are not liable for delayed or undelivered messages.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the categories of information described in our Notice of Privacy Practices exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Your use of the Site and our services is also governed by our Notice of Privacy Practices, which describes how we collect, use, and disclose your protected health information in accordance with HIPAA and applicable law. The Notice of Privacy Practices is incorporated into these Terms by reference and is available on the Site or by request.
The Site and its original content, features, and functionality — including text, graphics, logos, images, and software — are the property of Living Tree Medical Group or its licensors and are protected by U.S. copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any content on the Site without our prior written permission.
The Site may contain links to third-party websites or services that are not owned or controlled by Living Tree Medical Group. We do not endorse, and are not responsible for, the content, privacy policies, or practices of any third-party sites or services. Your interactions with any third-party site or service are solely between you and that third party.
The Site and its content are provided on an “AS IS” and “AS AVAILABLE” basis, without warranties of any kind, either express or implied. Any legal action or proceeding arising out of or relating to these Terms or the Site shall be brought exclusively in the state or federal courts located in Cache County, Utah, and you consent to the personal jurisdiction of such courts.

If a revision is material, we will make reasonable efforts to provide notice, such as by posting a notice on the Site or updating the Effective Date at the top of these Terms. Your continued use of the Site after any changes constitutes your acceptance of the new Terms.
If any provision of these Terms is held to be invalid or unenforceable by a court, the remaining provisions of these Terms will remain in effect.
